This document provides an information and resource guide on healthcare services and options for individuals who are exiting incarceration and seeking to re-enter the community.

Abstract

This document serves as a roadmap for formerly incarcerated individuals as they re-enter the community after imprisonment. It covers some immediate needs such as getting identification cards and other important documents, finding a job, obtaining food and transportation, as well as finding housing. The document seeks to help individuals take care of their health, including healthcare coverage and medication access, and stresses the importance of attending to mental and physical health needs. The guide is presented in four sections: identifying health needs, taking care of physical and behavioral health, and emergency and crisis information; things to know about health, with information on people who can help; application for health coverage, noting special considerations and information about, Medicaid, insurance, and Medicare; and how to use health coverage once enrolled, including a list of links and information resources.
