This manual presents the results of roundtable proceedings addressing effective discharge planning for inmates with HIV.
Abstract
This manual is the result of roundtable proceedings with experts in correctional- and community-based continuity of care and in department of corrections security to develop successful models for discharge planning and recommendations for initiating pre-release planning programs for inmates with HIV/AIDS. The manual begins with an overview of the proceedings and discusses key personnel involved in discharge planning: medical professionals, security professionals, case workers, and others such as chaplains, superintendents, and wardens. The third section of the manual offers guidance on designing a discharge planning model. The design process should take into account the needs of the inmates and the roles and responsibilities of the key players involved in the process. The fourth section of the manual provides information on resources that can be of use to recently released inmates and those in charge of the discharge planning model. Information that may be of assistance to inmates includes financial access to health care, community programs and services that provide assistance to recently released inmates, living situations that may be available in the community for recently released inmates, and 10 practical interventions for housing newly released inmates. The four appendixes contain copies of a discharge needs assessment form, a discharge checklist, a discharge medical/medication tracking sheet, and a patient medical information release form. Electronic versions of the appendixes are available on the CD-ROM included with the manual. References
