Following a review of definitions, research, and theory on narcissistic, antisocial, and borderline personality disorders, the need to reconceptualize these disorders on a behavioral continuum is discussed.
Abstract
Although the Diagnostic and Statistical Manual (American Psychiatric Association) defines these disorders as discreet but related, empirical research has shown difficulties in their differential diagnosis and has indicated that factors unrelated to overt behavior often are used in diagnosis. Yochelson and Samenow have placed all three disorders on a continuum of irresponsible behavior, while Bowen has defined irresponsible behavior in terms of a continuum of differentiation. Carillo offers a conceptualization of these disorders that combines these two perspectives in terms of the degrees of social self-centeredness and differentiation. Evidence supports the existence of a continuum of behavior and suggests that self-centeredness and differentiation are dimensions common to the three disorders. A clear and consistent conceptualization of personality disorders, such as that offered here, should result in more consistent correctional treatment and improve patient care.
