              This document outlines a format for a comprehensive emergency response analysis and the collection of significant data on aircraft accident emergency response services.
          Abstract
              Reports on incidents and accidents that involve fires on airplanes should include information on the origin of the fire, emergency exit data, weather conditions at the time of the fire, fire-fighting equipment used, injuries or death due to the fire, and flight crew's discovery of the fire. Sample report forms are provided. Regarding fuel, information should include the type of fuel on board at the time of the accident, a description of the fuel spill, and whether other flammable liquids, combustible metals, or combustibles were present. Other information that should be included: probable origin of the fire, damage to the engine, the condition of the aircraft's electrical circuits, and whether electrostatic sparks occurred. The progress of the fire should be reported, as well as its path and speed, and how it was extinguished. Emergency notification information should include: how long the after the accident was it discovered, whether the location of the accident was accurately described, how the fire alarm was transmitted, and the description of fire and evacuation conditions at the time of the arrival of the rescue units.
