The manual delineates the case management process at the New Hampshire State Prison and explains the functions and components involved in case management.
Abstract
Each inmate is assigned a case manager who is involved in the decision process from the inmate's admission until or even after release to the community. During the reception process, the case Manager, ensures that the inmate understands the 'inmate Manual,' monitors the inmate's mental and physical health needs, and identifies potential candidates for vocational rehabilitation. At the initial classification hearing, the case manager recommends a custody grade (medium or maximum custody) consistent with the required degree of supervision and assists in such decisions as the assignment of housing, special status, and work/training. Case managers base their decisions and recommendations on the case manager data base, which is a compilation of all relevant information at the time a particular case management contact takes place; it includes the inmate's psychological evaluation, social history, medical limitations, work history, and correctional history. The classification board has the authority to review the circumstances (e.g., custody requirements) of an inmate at any time. At such times, the case manager attends each hearing and facilitates the inmate's communication to the board. The case manager also cooperates with the inmate in cases of special classification board hearings, administrative procedures to bypass the board, inmate requests to appear before the board, and other procedures in the administration of an inmate's case. Appended are a summary of case manager procedures, an alcohol/drug counseling program, a biofeedback program, and a sexual offender program for the use of case managers. Forms used by case managers are provided.
