Abstract
During a two-year consultation, the intake interview was selected as a critical intervention point for rehabilitation services. The article describes a new intake form specifically designed for a county correctional facility, which is free of 'jargon,' concise, yet offering a thorough review of the relevant history. It also describes briefly how the information gathered can be used programmatically. Current research efforts using the form are briefly described. (Author abstract)
