FINDS THAT DEFICIENCIES IN PERFORMANCE BY PERSONNEL WERE THE MAJOR FACTOR EXPLOITED IN THE ESCAPE, WITH WEAKNESSES IN PHYSICAL FACILITIES AND MANAGEMENT BEING INVOLVED TO A LESSER DEGREE.
Abstract
FAILURE OF CORRECTIONAL OFFICERS TO CONDUCT SECURITY INVESTIGATIONS AND ADHERE TO PRESCRIBED SECURITY PROCEDURES IS CONSIDERED THE PRINCIPAL DEFICIENCY CONTRIBUTING TO THE ESCAPE. WHILE INADEQUACIES IN PHYSICAL FACILITIES IS NOT CONSIDERED TO HAVE BEEN MAJOR FACTOR IN THE ESCAPE, SUGGESTED IMPROVEMENTS COULD THWART AN OTHERWISE SUCCESSFUL ESCAPE ATTEMPT. A PROBLEM CONSIDERED FUNDAMENTAL WAS POOR OPERATIONAL CONTROL EXERCISED BY THE DIRECTOR AND SUPERVISORS AT ALL LEVELS. IT CONSISTED MAINLY OF A FAILURE TO EMPHASIZE THE IMPLEMENTATION OF STANDING ORDERS AND DIRECTIVES IN THE MIDST OF NEW PROGRAM DEVELOPMENT. --IN FRENCH AND ENGLISH
