Abstract
The manual's section on program components first presents a chart of the addiction services program structure which consists of four tiers: required substance abuse education, intensive outpatient treatment, day care, and residential treatment. The choice of program components is determined by the individual case management plan. The section on program components also contains forms for the program profile, the program component schedule, and program component description. The section on treatment documentation contains the case file model, the attendance log, the drug education roster, program forms for screening and an individual counseling request, and the monthly report for the management-information-system client tracking system. The training section presents the counselor certification plan, training record, and supervised practical training summary; counselor certification requirements; and staff development information. The program-management section provides an overview of treatment intervention, an outline of treatment intervention procedures, administrative procedures, and report forms for monthly program activity. A resource listing encompasses books, pamphlets, handouts, and videos.
