To complete your jurisdiction's registration you will need the following information:
- Jurisdiction DUNS Number
- Jurisdiction Employer Identification Number (EIN)
- Jurisdiction Address
- Primary Point-of-Contact Name, Position Title, Phone Number, Email Address, Agency Name, and Address
- Chief Executive Officer Name, Phone Number, and Email Address
- Financial Point-of-Contact Name, Phone Number, and Email Address
Registration should be completed by the jurisdiction's chief executive or authorized designee. If you are the designee, you will be acting on behalf of the jurisdiction's chief executive and must have the authority to do so. To begin registration select your state from the below list. If you do not have all the required information you may still begin the registration process. The system will allow you to return to registration process at any time once you have created your username and password.
